NBC Today and Roker fans have been on edge since November 2022 when Roker missed the show for a few days, only for it to turn out he was being treated for blood clots in his lungs. When he returned to Today on January 6, 2023, he revealed that his health issues had been considerably more severe. During the two months he was away he experienced two bleeding ulcers, internal bleeding which required surgery, colon resection surgery, and his gallbladder was removed.

In 2020, Roker was diagnosed with prostate cancer and had to have his prostate removed. He’s also had two knee replacements and a hip replacement. However, the health issue he’s best known for is his gastric bypass, which was performed in 2002. Before this, many associated Roker with his large figure, but since the surgery, he’s dropped to a healthy weight and even wrote a book about the experience called Never Goin’ Back: Winning the Weight Loss Battle For Good in 2013.
