The Health Department of the Government of the Canary Islands has launched a groundbreaking programme to guarantee the continuity of patient care following hospital discharge across the entire archipelago. Pilot Phase Expansion: The AP_Cuida2 programme, spearheaded by the General Directorate of Patients and Chronic Diseases of the SCS, was initially introduced in the Basic Health Zone of Tamaraceite, Las Palmas de Gran Canaria earlier this year. Now, this initiative is fully operational in the Health Areas of Gran Canaria, Lanzarote, Fuerteventura, La Gomera, and El Hierro, with ongoing efforts to complete its implementation in Tenerife and La Palma. Enhancing Patient Care: The programme's primary focus is to enhance personalised patient care, facilitate seamless coordination among healthcare providers, and highlight the invaluable role of nursing in improving overall patient well-being. Discharged patients under this programme receive regular follow-up calls from their assigned nurses at the healthcare center to ensure a smooth transition and continuity of care at home. Innovative Approach: AP_Cuida2 is lauded as an innovative initiative that streamlines post-hospital discharge patient care, significantly reducing hospital readmissions, emergency room visits, and medication-related issues. It particularly benefits patients with chronic conditions and the elderly, fostering a closer relationship of trust between healthcare providers and patients. Ensuring Care Continuity: The General Directorate of Patients and Chronic Diseases aims to bridge care continuity across different healthcare levels, ensuring easy access to essential patient information for both Primary Care and Hospital Care.
